9/30/2016 – 5/31/2018

Definition of terms (Glossary):

Contacted-Not Engaged Individual has been identified as homeless on the streets of Malibu and the Outreach Team has had at least one interaction with them. The number fluctuates from month to month. The reason for the fluctuation is not fully known at this time but may be due to homeless people ‘passing through’ Malibu. The greatest impact on the Malibu community is the number related to Engaged, below.
Repeat Contact – Not Engaged Individual has been identified as homeless on the streets of Malibu and the Outreach Team has had multiple contacts with them but individual is still unreceptive to interacting with the Team about needs or goals.
Engaged Level 1 The Outreach Team has had multiple contacts with the homeless individual who is now, or has been, at varying stages of accepting services.
Engaged Level 2 The homeless individual has achieved Level 1 Engagement AND is receiving additional intensive, street-based services from agency’s interdisciplinary teams (which include physicians, nurses, psychiatrists, mental health professionals, substance abuse experts and peers).
Disengaged Individuals who have not been contacted in over 3 months are considered disengaged.
Current Enrolled Client has consented to services, has been assigned to a Case Manager and has completed a goal plan. Individuals who have been disengaged are no longer considered enrolled.
Off Streets Client has been housed in interim housing or stable permanent housing and is no longer on the streets of Malibu.
Housing Voucher Issued Because many homeless individuals in Malibu do not have incomes that will allow them to afford market rate rents, one way for them to secure an apartment is to receive a government sponsored housing rent subsidy in the form of a “Voucher.”
Housed Client is in permanent stable housing either subsidized with a voucher or unsubsidized because they can afford the rent on their own or have been reunited with family or other shared housing.
